Wikipedia notes xylitol has a glycemic index near seven and minimal effects on blood glucose and insulin, which supports its role as a sugar substitute for people watching carbohydrates.

Manufacturers obtain xylitol by hydrolyzing plant xylan to xylose and hydrogenating it to xylitol, according to Wikipedia, and fermentation routes also exist though they are less economical.

Dental groups and reviews cited by Harvard Health and by a Cochrane review find evidence that regular low dose xylitol use, typically five to ten grams per day in gum, lozenges, or oral care products, can reduce tooth decay compared with no xylitol products.

Regulators have taken measured positions. Wikipedia records that the European Food Safety Authority allowed a health claim that xylitol gum lowers post‑prandial blood glucose and insulin responses and recognized a gum claim for reducing caries in children.

Safety signals are mixed. Both Harvard Health and Wikipedia warn that high intakes often cause gas, bloating or diarrhea. Harvard Health also cites an observational study in the European Heart Journal that linked higher xylitol levels to greater later risk of heart attack and stroke, a finding the source calls tentative and in need of more research.

Emerging Uses Benefits and Research Gaps
